Parent/Guardian Consent & Liability Waiver
Consent for your child to participate, plus acknowledgment of program terms and release of liability.
Medical & Emergency Contact Form
Medical details, emergency contacts, physician and insurance information, and treatment authorization.
Photo & Media Release
Grant or decline permission for photos, video, and likeness to be used in Girl, What? materials.
Transportation Permission Form
Permission for transportation to and from program activities, plus authorized pick-up persons.
Code of Conduct Agreement
Participant and parent/guardian agreement to the Girl, What? standards of respect and behavior.
